An NCR — non-conformance report — is the formal record raised when work, materials, or processes do not conform to the specification, the drawings, an Australian Standard, or the project’s quality requirements. What counts as a non-conformance

A non-conformance is any output that fails to meet a stated requirement: concrete that misses its specified strength, waterproofing installed without the specified membrane, work that skips a hold point in the inspection and test plan, or materials substituted without approval.

The trigger is the requirement, not the visible quality of the work. Work can look fine and still be non-conforming; that is exactly why the NCR exists as a formal record rather than a site conversation.

What a good NCR includes

An NCR is a record other people act on — the subcontractor rectifying it, the engineer approving a disposition, the client’s quality representative auditing it later. Vague NCRs create disputes; specific ones close.

  • A unique NCR number, the date, and who raised it.
  • The requirement that was not met: specification clause, drawing reference, standard, or ITP hold point.
  • A specific description of the non-conforming work with location and photos.
  • The proposed disposition: rework, repair, use-as-is with engineering approval, or reject.
  • Who is responsible for the rectification and the date it is required by.
  • Verification and sign-off once the disposition is completed.

NCR versus defect: same loop, different trigger

A defect is work that is faulty or incomplete — usually found on inspection walks and handled through the defect list. An NCR records a failure against a stated requirement and often carries a formal disposition process, including the possibility of accepting the work with engineering approval.

The loops overlap heavily: both need a structured record, photos, an owner, a rectification, and verified closure. Many items could be raised either way, and on most projects the NCR register and the defect list run side by side — NCRs for specification and process failures, the defect list for quality items found on inspection.

Closing the loop is what auditors check

An NCR register full of open items is worse than no register at all, because it documents known problems without documenting their resolution. Closure means the disposition was completed, verified, and signed off — with evidence.
